Collective Impact Team

Goals

Create community change to improve health outcomes and well-being by developing the capacity of our nonprofit organizations to support their work addressing social determinants of health in Rio Arriba County. We are committed to helping build a healthier community to support a stronger, more vibrant workforce. Healthy communities build healthy people who power healthy communities. This is the core of our collaborative work.

We will continue to develop intentional collaboration to create a strategy to bridge the work addressing adverse childhood experiences and social determinants of health. We will convene, listen, provide tools and resources, analyze data, and facilitate collective action among a committed set of partners.

UWNNM, in partnership with Los Alamos Community Foundation, will facilitate capacity building of local nonprofit, behavioral health and educational organizations, who provide critical services.

We will include a multigenerational component to support grandparents raising grandchildren and multigenerational families. UWNNM will also partner with Somos Un Pueblo Unido to better connect local community services with the immigrant community of northern New Mexico working toward social and economic justice.
